Two techniques that are applied separately or combined
There are two types of blepharoplastyς: upper and lower. If there are indications, both procedures can be performed in one surgery. After all, they are very quick procedures with short recovery time.

Upper blepharoplasty

The main indication for upper blepharoplasty is the removal of sagging skin at the upper eyelids, which sometimes become so “heavy” that they even interfere with vision.

With upper blepharoplasty:
  • Excess skin at the upper eyelids is removed
  • Oedema at the inner corner of the eyes or upper eyelids is relieved
  • Any accumulated fat is removed
Lower blepharoplasty

The main causes for bags and fat accumulation under the eyes are aging, weight gain, loss of skin elasticity and weakening of the supportive tissue. Sometimes under-eye bags may be related to medical conditions or heredity.

With lower blepharoplasty:

  • Under-eye bags are minimized or eliminated
  • Oedema and wrinkles under the eyes are minimized or eliminated
  • Dark circles are reduced

There are various techniques to perform blepharoplasty.

Surgery planning is of paramount importance, as it is the stage where the plastic surgeon decides where the incisions will be made, the amount of skin to be removed, whether fat removal is required and how it will be performed, etc. What is essential is to get a youthful and relaxed, perfectly natural look!

Upper blepharoplasty

At upper blepharoplasty excess skin and fat is removed through a delicate incision at the crease of the upper eyelid. It is essential to correctly recreate the crease of the eyelids, for the scar to be “concealed” in it. It is sutured with very delicate, non-absorbable sutures, which are removed after a week.
In addition, through upper blepharoplasty, the lash line can be slightly curved outwards, thus achieving the "mascara" effect.


Lower blepharoplasty

Lower blepharoplasty is considered to be a more challenging procedure than the upper one and the surgeon's experience can make the difference in the effects. Depending on the desired effects, lower blepharoplasty can be performed in the following ways:

1)    If there are only “bags”, this fat can be removed with an incision at the inner side of the lower eyelid (transconjunctival lower blepharoplasty) thus avoiding external incision and sutures.

2)    If there excess skin to be removed, the option of  the classic lower blepharoplasty is preferred. This is performed with an incision of 3-4 millimeters under the eyelashes, through which excess skin and “bags” are removed.

3)    By combining the two techniques.

Canthus is the ligament where the upper and lower eyelids meet, either inner or outer. The position of canthus and the strength of the support it provides are important for both the function and the look of the eyelids. More specifically, the outer canthus is a little higher than the inner one and this creates a tilt of the eye upwards, known as “almond-shaped eye”.

If, over time, canthus loses its strength, this point will loosen and the eyelid will move downwards, making the look sad.

This problem is corrected with canthopexy or canthoplasty.

At canthopexy the surgeon elevates the corner of the eye and supports it with sutures to prevent its new downward movement.

At canthoplasty, which is considered to be a challenging procedure, the outer canthus is detached and repositioned to a new, higher location. Canthoplasty is often applied to patients whose eye shape is downward by nature.  The success of the effects is highly dependent on the plastic surgeon’s competence and experience.

Local anesthesia is the main option, for both upper and lower blepharoplasty.

In addition, eyelid procedures may be performed under conscious sedation and under general anesthesia. Anesthesia is a topic you can discuss in detail with your doctor.

Blepharoplasty is a procedure that does not cause any inconvenience to the patient. Some patients develop bruises and mild edemas, while there is limited or no postoperative pain. No wonder blepharoplasty has been described as the “Weekend Surgery”; if performed on a Friday, you can cautiously return to some of your activities on Monday.  

Sutures are usually removed after 5-7 days; then you return to your social and professional activities as usual.  

What is very important after blepharoplasty is to avoid exposure to the sun and to use face sunscreen daily.

If your eyes are covered by your droopy eyelids or if you have pronounced under-eye bags, you are suitable for blepharoplasty. The patients interested in this procedure are between 40 and 60 years of age, which is not necessarily the norm.
